Reappointment Forms

Reappointment forms for next year (AY 2008/09) are now available.

Please complete and return these forms to Marci no later than Friday January 18, 2008.

There are three versions of the form, corresponding to the three stages of our graduate program:

Level 1. For students who have not yet completed all comp exam requirements
Level 2. For students who have fulfilled all comp exam requirements, but have not yet passed the Preliminary Exam
Level 3. For students who have passed the Preliminary Exam

All versions are in two parts, with the same Part 1 but different, level-specific -Part 2:

Part 1 covers appointment/financial aid requests, and
Part 2 asks for basic information on your academic progress.

Part 2 of the form is designed to help us keep more accurate records and to help you maintain a clear idea of how well you are advancing towards your goal of a doctorate in mathematics. The Departmental expectations are described in Section 6.5 of the Graduate Handbook, or can be found online at

The timeline described there gives the minimal progress required for reappointment as a TA. Slow progress, as measured by these guidelines, does not necessarily preclude continued financial support; we recognize that each person's circumstances are unique and that general guidelines cannot be rigidly applied. However failure to meet the guidelines is definitely cause for concern. If you have any questions about your progress, I encourage you to discuss them with me and/or your advisor as soon as possible.

Please note:

  • All students are required to submit the two-part form
  • Part 2 must be signed by your Thesis Advisor (if you have one) or your assigned faculty advisor (if you don't yet have a Thesis Advisor). It goes without saying that it's a good idea to first discuss the contents of the form with your advisor!
  • All international students must satisfy the English proficiency requirement in order to be eligible for any TA appointment.
